The client functions in the healthcare and insurance space. The client work closely with insurance providers, doctors, and elderly patients. A major part of insurance costs was being spent on serving senior citizens due to late diagnosis.

The client wanted a system that could support preventative care. The goal was to keep elderly patients healthier through regular checkups. This goal also involved reducing long term costs for insurance providers.

Challenges

Insurance companies were enduring high medical costs for senior citizens. The reason for these high medical costs were late detection of health issues.
Many elderly patients did not go for regular checkups, which led to serious health complications.
There was no systematic approach to track patient health history across doctors and hospitals.
Doctor performance was difficult to measure due to lack of consolidated data.
The overall system lacked reminders, monitoring, and performance visibility.

Goals & Objective

The objectives of the project were clear.

Encourage regular health checkups for elderly patients.
Maintain up to date medical records for patients.
Monitor and measure doctor performance based on treatment outcomes.
Reduce preventable health risks and lower mortality rates.
Control long term healthcare costs for insurance providers.

Solutions

Acme One developed a combined healthcare application. This healthcare application connected patients, doctors, and insurance stakeholders. The system captured complete patient medical records. These medical records include visits, signs, vitals, blood pressure, diabetes levels, lab tests, procedures, and medications. Each patient was associated with a primary care provider. Patients would first visit their primary doctor and then be referred to specialists when needed. Doctor details such as hospital or clinic information were also maintained. The application monitored doctor performance. The application is monitored by tracking treatment plans, recovery time, and medicines prescribed. Performance was evaluated based on ACO (Accountable Care Organization) rules. Automated reminders were sent to patients to encourage regular checkups and follow ups. Reports and operational summary were generated using SharePoint and Power BI charts. These reports gave clarity into doctor outcomes and patient health trends.

Implementation

The project was implemented in phases.

A centralized patient record system was developed using ASP.NET MVC and SQL Server.
Doctor and patient workflows were mapped into the system.
Performance measurement logic based on ACO rules was implemented.
Reminder mechanisms were added to notify patients about routine checkups.
Reporting dashboards were created using SharePoint Power BI charts.
